This document contains important general information and deadlines for all those attending FLFPSP State Competition 2010
These documents may also be distributed at the District Competitions
If you did not attend the district competition you must download NOW in order to proceed with your registration


REQUIRED  CONTRACTS: Mandatory for every participant
Students' and parents' signed behavior contracts and publication releases must be completed and sent with your state registration forms
NOTE:  Original signed contracts may be sent to Dianne Zahrly or may be brought to the State competition.
Remember: BOTH forms are mandatory for participation not only for state competition but are also required for the International Conference.

Please fax or mail all of the appropriate registration forms from your school at one time to prevent errors.
Download those that apply to your school and send to Dianne Zahrly as directed in the general information packet.

Form 1 is a summary and MUST accompany all registration forms for your school


Registration Form 2 for Schools/coaches with qualified teams invited to FLFPSP State Competition


Registration Form 3 for coaches without advancing team attending to evaluate with an alternate and individual


Registration Form 4 for additional evaluators without team


Form 5 Volunteers or Additional Evaluators - not previously registered


Registration Forms 6 and 6A Team and Individual CmPS registration
Use both forms if your school is only registering CmPS teams or CmPS Individuals
If your school has registered participants in Team, Individual, or Alternate Competition use only Form 7A (below) with Form 1 (Summary Sheet)


Registration Forms 7 and 7A Group Scenario Registration
Use Both forms if your school is only registering Group Scenario Writers
If your school has registered participants in Team, Individual, or Alternate Competition use only Form 7A with Form 1 (Summary Sheet)


Form 8 Circus Maximus Registration
All teams must be pre registered. Download if your students will be participating
